Insurance Policy & Self-Submission Process

At CenteRD Nutrition, we are dedicated to providing individualized, compassionate care to support your health journey. We understand that navigating the financial aspect of healthcare can be challenging, and we want to be transparent about our policies. At this time, CenteRD Nutrition does not accept insurance directly for services. However, we recognize that many of our clients may wish to use their insurance benefits and can assist with self-submission of claims to your provider.

While every insurance plan varies slightly, below is a general step-by-step guide to help you submit your claim:

Step-by-Step Guide for Self-Submitting Insurance Claims:
  1. Obtain a Superbill: After your session(s), we will provide you with a detailed receipt, known as a “superbill,” which includes all necessary information your insurance provider will need (CPT codes, diagnosis codes, fees, and practitioner details).
  1. Check Your Insurance Policy: Before submitting, it’s a good idea to review your policy or contact your insurance provider to confirm your coverage for out-of-network nutrition services. Ask if a doctor’s referral is required or if you need to meet a deductible before reimbursement.
  1. Complete Your Insurance Claim Form: Most insurance companies have a claim form available on their website. You will need to fill out this form and attach the superbill. Be sure to include your insurance ID number and other requested personal information.
  1. Submit Your Claim: Submit your completed claim form and superbill to your insurance provider. This can usually be done online through your insurance provider’s portal, by email, or through the mail, depending on your carrier’s instructions.
  1. Follow Up: Insurance claims processing times can vary. After submission, follow up with your insurance provider to check on the status of your claim and ensure it’s being processed.
Important Reminders

Coverage & Reimbursement: Each insurance carrier and plan differs in what they cover and how much they reimburse for out-of-network services. We encourage you to contact your insurance provider to understand your benefits.

Out-of-Network Benefits: If your plan includes out-of-network benefits, you may be eligible for partial reimbursement. Please be aware that submission of a claim does not guarantee reimbursement.
